Healthcare Providers and Facilities - Decision Tree for 2009 H1N1 Vaccination

Purpose of This Document:

To provide a decision tool for providers and healthcare facilities including:

  • Provider offices (pediatricians, family practice physicians, internists, primary care providers, obstetricians/gynecologists)
  • Federally Qualified Health Centers and “look-alikes”, Community health clinics, Urgent Care clinics, Retail-based clinics
  • Hospitals
  • Long-term care facilities

Target Audience:

Physicians, nurses, office/facility managers, infection control coordinators, and anyone responsible for carrying out 2009 H1N1 Influenza vaccination in healthcare settings. This document can also be used by state and local public health planners to assist with vaccine distribution and coordination of related community mitigation.
