Community Transformation Grants (CTG) States and Communities Program Descriptions

CDC awarded CTG funds to 61 state and local agencies and organizations to support efforts to make healthy changes in their communities.

Awards were made as follows:

• 29 to large counties—those with a population over 500,000.
• 10 for entire states—in cases where the state does not have a large county.
• 14 for states minus their large counties.
• 7 to tribes.
• 1 to a territory.

The 61 awardees received one of two types of awards:

• Implementation awards help communities plan and carry out proven programs and strategies.
• Capacity-Building awards help communities build the skills and support needed to make lasting local changes.

Implementation Awards

• The Southeast Alaska Regional Health Consortium receives $499,588 to serve a population of about 72,000 in Alaska. Work focuses on tobacco-free living, active living and healthy eating, and quality clinical and other preventative services.
• The Public Health Institute in California receives $5,926,365 to serve the state minus large counties. The population is about 5,900,000 including a rural population of more than 833,000. Work focuses on tobacco-free living, active living and healthy eating, quality clinical and other preventive services, and healthy and safe physical environments.
• The County of San Diego Health and Human Services Agency in California receives $3,053,793 to serve the large county of San Diego. The population is about 3,095,000. Work focuses on tobacco-free living, active living and healthy eating, quality clinical and other preventive services, social and emotional wellness, and healthy and safe physical environments.
• The San Francisco Department of Public Health in California receives $815,358 to serve the large county of San Francisco. The population is about 805,000. Work focuses on tobacco-free living, active living and healthy eating, quality clinical and other preventive services, and healthy and safe physical environments.
• The Los Angeles County Department of Public Health in California receives $9,848,011 to serve the large county of Los Angeles. The population is about 10,400,000. Work focuses on tobacco-free living, active living and healthy eating, quality clinical and other preventive services, social and emotional wellness, and healthy and safe physical environments.
• The Denver Health & Hospital Authority in Colorado receives $610,345 to serve the large county of Denver. The population is about 600,000. • The Broward Regional Health Planning Council in Florida receives $1,766,476 to serve the large county of Broward (including the city of Fort Lauderdale). The population is about 1,700,000. • The Illinois Department of Public Health receives $4,781,121 to serve the state minus large counties. The population is about 4,800,000 including a rural population of more than 1,600,000. • The Iowa Department of Public Health receives $3,007,856 to serve the entire state. The population is about 3,000,000 including a rural population of more than 1,300,000. Work focuses on tobacco-free living, active living and healthy eating, and quality clinical and other preventive services.
• The Louisville Metro Department of Public Health and Wellness in Kentucky receives $721,594 to serve the large county of Jefferson. The population is about 740,000. • The Maine Department of Health and Human Services/Maine CDC receives $1,318,301 to serve the entire state. The population is about 1,300,000 including a rural population of over 545,000. • The Maryland Department of Health and Mental Hygiene receives $1,945,289 to serve the state minus large counties. The population is about 1,900,000 including a rural population of more than 300,000. • The Massachusetts Department of Public Health receives $1,505,006 to serve the state minus large counties. The population is about 1,574,000 including a rural population of 27,000. • The Massachusetts Department of Public Health receives $1,574,982 to serve the large county of Middlesex (including the cities of Lowell and Cambridge). The population is about 1,500,000. • The Sault Ste. Marie Tribe of Chippewa Indians receives $500,000 to serve an estimated tribal population of more than 176,000 in Michigan. • The Minnesota Department of Health receives $3,603,724 to serve the state minus large counties. The population is about 3,600,000 including a rural population of over 1,300,000. • The Hennepin County Human Services and Public Health Department in Minnesota receives $1,156,212 to serve the large county of Hennepin (including the city of Minneapolis). • The Mid-America Regional Council Community Services Corporation in Missouri receives $705,708 to serve the large county of Jackson (including the city of Independence and portions of Kansas City). • The Montana Department of Public Health and Human Services receives $769,195 to serve the entire state. The population is about 974,000 including a rural population of 630,000. • The Douglas County Health Department in Nebraska receives $510,199 to serve the large county of Douglas (including the city of Omaha). The population is about 510,000. • The New Mexico Department of Health receives $1,500,000 to serve the state minus large counties. The population is about 1,300,000 including a rural population of more than 673,000. • The Fund for Public Health in New York receives $8,391,881 to serve the large counties that comprise the City of New York. The population is about 8,390,000. • The Oklahoma City-County Health Department in Oklahoma receives $716,704 to serve the large county of Oklahoma. The population is about 718,000. • The Texas Department of State Health Services receives $10,000,000 to serve the state minus large counties. The population is about 10,000,000 including a rural population of more than 2,900,000. • The Tacoma-Pierce County Health Department in Washington receives $796,836 to serve the large county of Tacoma. The population is about 814,000. • The University Health Services at the University of Wisconsin-Madison is receiving $4,695,253 to serve the state of Wisconsin minus large counties. • The Vermont Department of Health receives $621,760 to serve the entire state. The population is about 620,000 including a rural population of more than 413,000. • The Washington State Department of Health receives $3,256,347 to serve the state minus large counties. The population is about 3,250,000 including a rural population of more than 800,000. • The West Virginia Bureau for Public Health receives $1,883,603 to serve the entire state. The population is about 1,800,000 including a rural population of 800,000. Capacity-Building Awards

• The Yukon-Kuskokwim Health Corporation receives a $193,340 planning award to build capacity to support healthy lifestyles among an estimated tribal population of 25,000 in Alaska. Work targets tobacco-free living, active living and healthy eating, quality clinical and other preventive services, and healthy and safe physical environments.
• Ventura County Public Health in California receives a $481,036 planning award to build capacity to support healthy lifestyles in the large county of Ventura (including the city of Oxnard). The population is about 823,000. Work targets tobacco-free living, active living and healthy eating, quality clinical and other preventive services, social and emotional wellness, and healthy and safe physical environments.
• The County of Kern’s Public Health Services Department in California receives a $416,577 planning award to build capacity to support healthy lifestyles in the large county of Kern (including the city of Bakersfield). • The Fresno County Department of Public Health in California receives a $499,695 planning award to build capacity to support healthy lifestyles in the large county of Fresno.
