Methods for State Oral Health Surveys that began data collection in 2002

Massachusetts
The survey was conducted during the 2002-2003 school year. The sampling frame consisted of public elementary schools with 20 or more students in third grade. With implicit stratification by percent of students considered to be low-income, 102 schools were randomly selected of which 95 agreed to participate. Dental hygienists completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. The response rate was 53%. The estimates are presented adjusted and unadjusted for nonresponse.
Nevada
The survey was conducted during the 2002-2003 school year. The sampling frame consisted of all public elementary schools with a 3rd grade enrollment of 20 or more. The sampling frame was stratified by region with implicit stratification within each region by percent of students eligible for the free/reduced school lunch program (FRL). Fifty-one schools were selected and 50 agreed to participate. Positive consent was used with a response rate of 46%. Volunteer dentists completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. In the state and in the sample, 39% of students were eligible for FRL; 45% of the students participating in the survey were eligible for FRL. The data were self-weighting and the estimates are presented adjusted and unadjusted for nonresponse.
Oklahoma
The survey was conducted during the 2002-2003 school year. A two-stage sampling procedure was used to select a stratified random sample of 3rd grade students in public and private elementary schools. Within each of the state's six regions, six elementary schools were randomly selected then one classroom within each school was randomly selected. Two dentists completed all the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. Active parental consent was used and the overall response rate was 74%. The data were weighted to account for the survey design. The estimated were not adjusted to account for nonresponse.
South Dakota
The survey was conducted during the 2002-2003 school year. The sampling frame consisted of all public, private and Bureau of Indian Affairs elementary schools with a 3rd grade enrollment of 3 or more. With implicit stratification by percent of students eligible for the free/reduced school lunch program (FRL), 36 schools were randomly selected. Thirty-five schools agreed to participate. Volunteer dentists completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. Of the students selected, 71% responded. The percentage of children in the sample eligible for FRL was 38% while 41% of students in the state were eligible for FRL. The data were self-weighting and the estimates are presented adjusted and unadjusted for nonresponse.
Vermont
The survey was conducted during the 2002-2003 school year. In Vermont there are 211 elementary schools with at least 20 children in 1st, 2nd, and 3rd grade. An electronic list of the 211 schools was ordered by free/reduced lunch participation and 23 schools were randomly selected. Of these, 22 schools agreed to participate. Of the 1,830 children enrolled in the 22 schools, 1,238 (68%) received a screening. Diagnostic criteria were comparable to ASTDD’s 1999 Basic Screening Survey. The percentage of children in the sample eligible for the free/reduced school lunch program was the same as that in the whole state (31%). The estimates are presented adjusted and unadjusted for nonresponse.
