Adult Dosing

• IV or PO: 200 mg initial dose, then 100 mg q12h.

Pediatric Dosing

• Avoided in children < 8 d/t calcium deposition in teeth
• Children >8 years: PO, IV: 4 mg/kg/dose for 1 dose; Then 2 mg/kg/dose every 12 hours
