Wheezing or course breath sounds?
Yes
- Suction and give epinephrine 1:1000 3mL nebulized
Evidence of Stridor?
Yes
- Give epinephrine 1:1000 3mL in nebulizer
No
- Reassess continuously during transport for respiratory distress
Ongoing Respiratory Distress?
Yes
- Give epinephrine 1:1000 3mL in nebulizer
- Immediately assess for respiratory failure.
No
- Reassess continuously during transport
- Further orders per Online Medical Control
Signs of respiratory failure?
- Increased decreased or no respiratory effort?
- Poor or absent air movement?
- Tachycardia (early sign) or bradycardia (late sign)?
- Changes in level of consciousness?
No
- Continue to reassess patient
Yes
- Perform BVM Ventilation
- Normal saline (weight *20)ml/kg bolus IV/IO
Is BVM effective?
If BVM is not effective, perform the following:
- Reposition the airway
- Check correct mask size
- Check O2 source
- Place OPA
- Assess for gastric distention
Yes
- Transport, monitor and continue to reassess
No
- If continued failure to ventilate or oxygenate with BVM, then place endotracheal tube or supraglottic airway
- Contact medical control for further orders
