- 25-200mg PO (weekly or daily, depending on indication), in combination with other antimicrobials
Pediatric Dosing
- 1-3mg/kg PO

Contraindications
- Allergy to class/drug
- megaloblastic anemia due to folate deficiency

- Pancytopenia, leukopenia, thrombocytopenia, megaloblastic anemia
