How can I request a select agent?
Follow the steps below to request a select agent:
- Register with the Select Agent Program (SAP).
- Fill out the SADA application.
- Send the completed SADA application and Certificate of Registration and the Transfer Authorization by the Select Agent Program (with SAP transaction ID) parts 1 and 2 to the SADA administrator.
- Wait for a response from the SADA Executive Committee regarding their decision to grant your request for the select agent.
- Pay a user fee, in advance of shipment and after application approval, that includes costs for materials, handling, and shipping.

What is the SADA application process?
Follow the steps below to complete the SADA application process:
- Download and print the SADA application form [PDF - 84 KB].
- Submit the documents listed below. All applications must contain original, ink signatures. The SADA program cannot accept electronic or faxed applications.
- Section 1. Provide the required information.
- Section 2. Initial where indicated. If you are not a United States Government employee, you must complete either the Standard Indemnification Agreement or the State Institution Compliance Agreement provided with the application packet as appropriate for your institution.
- Please Note: If your institution cannot agree to the terms of either indemnification agreement, you are not eligible to receive select agents from NCEZID. The form must be signed by the applicant and countersigned by an official capable of legally binding the institution (e.g., president, vice-president, dean, provost, corporate officer).
- Section 3. Complete the enclosed Biographical Sketch form or attach a brief curriculum vitae. A Biographical Sketch from a recent NIH grant proposal is also acceptable.
- Section 4. Research Focus. Limit to a maximum of one page in length. Complete or attach abstract from grant proposal.
- Completed Certificate of Registration and the Transfer Authorization by the Select Agent Program signed by local Responsible Official with SAP transaction ID.
Why am I required to submit a copy of the Certificate of Registration and the Transfer Authorization by the Select Agent Program along with the SADA application?
By submitting a copy of the Certificate of Registration and the Transfer Authorization by the Select Agent Program we can verify that you have taken the necessary preliminary steps for the transfer of a select agent.
What is the difference between the Select Agent Program (SAP) and the Select Agent Distribution Activity (SADA)?
The Select Agent Program is a national program responsible for registering and certifying laboratories for possession, use and/or transfer of select agent materials. The Select Agent Distribution Activity is an NCEZID office responsible for facilitating the evaluation of requests and the distribution of select agents from NCEZID to qualified users.
I filled out a Material Transfer Agreement (MTA) when I requested select agents in the past. Do I need to fill out the SADA application in addition to an MTA?
The SADA application includes an updated version of the MTA and is specific to the transfer of select agents. Therefore, it is NOT necessary to submit the previous version of the MTA in addition to the SADA application.
What are the costs associated with requesting a select agent?
Who is eligible to apply for the SADA application?
You must be a Principal Investigator, Laboratory Director or equivalent (public or academic institution), or a Director of Research or equivalent (private or for-profit institution). The requester must be registered with the Select Agent Program for use of the select agent at the site and facility where the research will take place.
Who is required to complete the SADA application?
ALL scientists requesting select agents from the NCEZID are required to complete the SADA application.
When will I be notified of my application's acceptance/rejection?
After the Executive Committee's review process, the requester will be contacted regarding their decision via mail and email. This is usually within a two-week period after receipt of application.
I have received approval for a select agent. How long until it is shipped?
The estimated time between when the request is approved and when the select agent is shipped is two to four weeks.
What is the maximum quantity of a select agent I am allowed to obtain?
Each requester is allowed to receive only one ml or prepackaged panel of strain of virus, etc., of a select agent.
I need to request two different select agents? Do I need to fill out applications and proposals for each select agent?
Yes. An individual SADA application must be filled out for each select agent requested. Each requires a separate MTA and involves different subject matter experts.
I already have an application on file for a select agent, but I want to request more of the same select agent. Do I need to submit an additional application?
For requests of the same select agent for the same project, submission of page 1 only of the SADA application is required. Requests for additional material will be evaluated on a case by case basis.
