The Division’s three grantee awards support states, large and small cities and counties, tribal organizations, universities, and community and healthcare organizations.
State Public Health Actions (1305)
This award funds statewide initiatives to prevent, manage, and reduce chronic diseases—including childhood and adult obesity, diabetes, heart disease, and stroke.
High Obesity
This award funds land grant colleges and universities to work with cooperative extension and outreach services in states with counties that have more than 40% prevalence of adult obesity.
Racial and Ethnic Approaches to Community Health (REACH)
REACH is a national to reduce racial and ethnic health disparities through local, culturally appropriate programs among African Americans, American Indians, Hispanics/Latinos, Asian Americans, Alaska Natives, and Pacific Islanders.
