Individuals at risk for type 2 diabetes may be more likely to join a CDC-recognized diabetes prevention lifestyle change program if their health care professional recommends it.

Individuals at risk for type 2 diabetes may be more likely to consider a lifestyle change program if their health care professional recommends it. In addition, the recognition program requires that at least 50% of participants be diagnosed with prediabetes through blood testing (or have a history of gestational diabetes); this is often done by a health care professional.

The new PreventT2 curriculum comes with ready-to-use, and adaptable promotional materials that can be easily modified to add your organization logo and contact information.

CDC provides resources for encouraging health care professionals to refer at-risk patients to your lifestyle change program. These resources will help you educate health care professionals about the program and offer them tools for easy referrals.

Also consider working with local medical assistance programs or public health departments to promote the lifestyle change program at area clinics.
