NHIS Questionnaires Available on the Internet

Questionnaires from the National Health Interview Survey (NHIS) are available for viewing and/or downloading via the Internet as follows:

1962 - 1996:

For each NHIS data year from 1962 to 1996, one of the documents that was released was called "Current Estimates from the (National) Health Interview Survey". These Current Estimates documents contained tables for each year's data. In addition, Appendix III in each Current Estimates document contains an image of most or all of the questionnaires used for that data year. The Current Estimate documents were released as part of the Series 10 group of documents from the National Center for Health Statistics (NCHS). The full list of Series 10 reports can be seen at the NHIS home page and, under the heading "Reports", click on "Vital and Health Statistics, Series 10 Reports (including Summary Statistics Reports)". * 1968: Four different questionnaires were used during the July 1967 - June 1968 (fiscal year 1968) and July 1968 - December 1968 (transition to calendar year) periods. Series 10, No. 60 contains one of the 1968 NHIS person approach questionnaires, dated 4-25-68, which was used during the July-December 1968 period. Another of the 1968 NHIS person approach questionnaires, dated 5-19-67, can be found as appendix IV to an NCHS publication titled "Interviewing Methods in the Health Interview Survey [PDF - 6 MB]". This questionnaire was used during the July-December 1967 period.

Two other NHIS questionnaires used during the July 1967 - June 1968 period (fiscal year 1968 condition approach questionnaire, person approach questionnaire dated 12-1-67, used January-June 1968) are available.

** 1986: Please note, the questionnaire in Appendix III of Series 10, Number 164, is the 1985 questionnaire, not the 1986 questionnaire, after page 1. The complete 1986 questionnaire is available here.

1997-present:

For NHIS data years from 1997 to the present, questionnaires can be obtained by going to the NHIS data page. Then, under the desired data year, click on the category that includes Survey Questionnaires. The questionnaires will all then be shown as documents that begin with the letter 'q'.
