The map shows that pockets of counties with high heart disease hospitalization rates – meaning the top quintile - are located in Texas, Colorado, Louisiana, Pennsylvania, Ohio, Michigan, Illinois, West Virginia, Kentucky, Tennessee, Alabama, North Carolina, Arkansas, and Missouri.
